Who is on the Home Health care team?

Before someone receives Home Health Care a specific plan of care is designed for that individual by our Professional Staff of Health Care Experts and your family physician. The members of our Home Health Staff include

Registered Nurses (R.N.) who provide skilled observation and complex care; R.N's will also teach the patient and family to care for the patient in the home and report significant observations to the physician.

Licensed Practical Nurses will provide routine nursing care.

Home Health Aides will help the patient achieve greater independence of daily activities. This may mean assisting with bathing, walking, dressing, and eating.

The Speech Pathologist is also a member of our Home Health Care Team. She will work with the patient and family, to improve communication skills.

Our Physical Therapists can offer the home bound patient professional treatment designed to renew or increase mobility. The Physical Therapists will also teach exercises for muscle strength.

Our Occupational Therapist can work with patients needing help in activities of daily living, to promote independence despite health problems.

Medical Social Services is provided by a social worker. And as part of our team they provide counseling,aide in decision making and long range and community resource planning

How is Home Health Care paid for?

Home Health Care is paid for by a variety of sources. Both private and public insurance programs cover most Home Health Care costs. The major sources of payment for Home Care are Medicare, Medicaid, Private Health Insurance, Worker's Compensation and HMO's.
